If you have already applied for an undergraduate or graduate program at the University of Kentucky you should not fill out this form.

Please have the following documents accessible before applying. After you submit your application you will get an email requesting the following additional documents:

A copy of the biographical information page of your passport, (and your family members' if applicable).
Proof of funding. For 2021-2022: Your bank statement must show at least $29,039 for one year of study. You or your sponsor should send a bank letter that is not more than 120 days old and is written in English, which verifies these funds.
- Self/Family Sponsored: a bank statement
- Government Sponsored: sponsorship letter
- Company Sponsored: letter from the company, and a bank statement
Unofficial copy of an English Proficiency Exam (TOEFL, IELTS, Duolingo)

Your application will not be considered complete until we receive all supporting documents.
